Work permit application - print out form, fill out your portion, have your supervisor fill out their portion, and then have your parent sign. Bring the completed form to Santiago for the Work Permit to be typed. Mrs. Bayless is available Mondays & Wednesday from 2:00 p.m. to 5:00 p.m or Tuesdays, Thursdays & Fridays from 10:00 a.m. to 1:00 p.m.
