Work permit application - print out form, fill out your portion, have your supervisor fill out their portion, and then have your parent sign. Bring the completed form to Santiago for the Work Permit to be typed. Mrs. Bayless is available Monday - Thursday from 9:15 a.m. to 1:00 p.m. during the school session. During the summer please bring to the front office.
